Meet The Team
Dr. Peter Klim, DO
Dr. Peter Klim is a double board-certified anesthesiologist and pain medicine specialist. He has been treating patients with chronic pain for over 20 years while educating them on the benefits of making healthy lifestyle changes. His passion for improving the lives of his patients led him to the world of anti-aging and wellness treatments.
While still practicing pain medicine at Medical Pain and Spine Care, Dr. Klim started Revitalyze Aesthetics & Wellness with the goal of helping patients uncover their most satisfying lives through the right anti-aging and wellness therapies.
Emily Hackett, FNP-C
Having worked as a registered nurse at IU Methodist and IU North Hospitals, Emily Hackett went on to complete her Advanced Degree in Nursing from Ball State University.
Emily specializes in skin care and aesthetics services. She is a certified injector of Botox® and dermal fillers.
At Revitalyze, she also administers InMode’s Morpheus8 Face and Body treatments, Lumecca IPL photofacials, and Diolaze laser hair removal to help patients look and feel their radiant best.

Haylee Goad, NP-C
Haylee Goad brings years of medical experience to Revitalyze. Haylee completed her BSc in Nursing at Ball State University, followed by an MSc in Nursing at Northern Kentucky University.
In 2020, Haylee received the prestigious Daisy Award for her extraordinary nursing services and impact on patient lives.
At Revitalyze, Haylee specializes in administering InMode’s Morpheus8 Face and Body treatments, Lumecca IPL photofacials, Diolaze Laser Hair Removal, and Botox injections.
